If you are aware of any violation of the following corporate governance rules, committed by any our directors, officers or employees, related to the following:

Violation of the Corporate Governance Rules
Violation of the Code of Ethics and Company Code of Employee Discipline or other similar codes
Financial Malfeasance including those relating to financial fraud and questionable accounting and auditing matters

We collect your name, address, employee number, job title/position, company/office, contact number, email address, and signature solely to facilitate action on the Violation of Corporate Governance Rules and/or Retaliation Protection report you have submitted. These are accessed by Corporate Governance Office who hold such information under strict confidentiality. These are retained while any of the involved parties is employed in Meralco and shall be disposed of accordingly thereafter.

Portion of Los Baños – Bay National Highway from Lopez Ave. to and including University of the Philippines – Los Baños Campus and Trace Computer College; Sitios Maligaya, Pag-asa and Sto. Niño in Bgys. Batong Malake, San Antonio and Maahas.

Portion of PNR Rail Road Track and IBP Road from Los Baños – Bay National Highway to San Luis Road in Bgys. San Antonio, Maahas and Putho – Tuntungin.

Along San Luis Road from IBP Road to and including GK – Los Baños Ville, Collegeville Subd., Dairyville Subd., Hillside Village, Pleasant Village and Villa Carangal; Labadan, Apitong, Kamagong and Yakal Sts. in Bgy. Putho – Tuntungin.

Portion of Lopez Ave. from Los Baños – Bay National Highway to and including Lopez Subd., Umali Subd., Sta. Fe Subd., Faculty Village and Demarces Village; El Danda and Bangkal Sts. in Bgy. Batong Malake.

REASON: Preventive maintenance and testing works inside Meralco - Los Baños substation.
