If you are aware of any violation of the following corporate governance rules, committed by any our directors, officers or employees, related to the following:

Violation of the Corporate Governance Rules
Violation of the Code of Ethics and Company Code of Employee Discipline or other similar codes
Financial Malfeasance including those relating to financial fraud and questionable accounting and auditing matters

These are accessed by Corporate Governance Office who hold such information under strict confidentiality. These are retained while any of the involved parties is employed in Meralco and shall be disposed of accordingly thereafter.

Portion of Gen. Tirona Highway from Meralco – Bacoor substation along Aguinaldo Highway to Gen. Evangelista St. including Patricia Executive Village in Bgys. Habay I & II and Mabolo I & III in Bacoor City.

Portion of Daang Bakal St. from Gen. Tirona Highway to and including Gahak Road, Tramo, Aroma, Bautista and Binakayan Sts.; Sunset Meadow Subd. and Fatima Compound; and Binakayan Public Market in Bgys. Gahak, Marulas and Tramo-Bantayan in Kawit.

Portion of Congbalay Road from Gen. Tirona Highway in Bgy. Balsahan-Bisita to and including Island Cove Hotel & Leisure Park, Covelandia Island, DENR Bldg., Our Lady of Fatima Catholic School and Child Development & Guidance Center; Bgys. Pulvorista, Congbalay-Legaspi, Samala-Marquez and Binakayan-Aplaya; and portion of Bgy. Binakayan-Kanluran I in Kawit.

REASON: Replacement of facilities and reconstruction of primary line in Bgys. Tramo-Bantayan and Pulvorista in Kawit, Cavite.
